Christopher Ocón Memorial Baseball | Onward! A Legacy Foundation

This fund was created to allow Montezuma County youth whose family is experiencing financial hardship to play baseball or softball with the Southwest Colorado Youth Baseball and Softball league (excluding select/travel teams) by covering 1/2 of the registration fees or by providing up to $50 towards the cost of equipment.

This scholarship is to assist low-income families with the costs of registering or purchasing equipment for participation in youth baseball programs. The scholarship will pay for ½ of the cost of registration or provide up to $50 towards the purchase of equipment.

IMPORTANT INFORMATION

Applicants must be residents of Montezuma County and provide a physical address.
Scholarships are available on a first come, first serve basis for qualified applicants. You will receive a letter of approval, denial or request for more information.
If you are awarded a scholarship, Onward! A Legacy Foundation will issue payment for 1/2 player registration fees on the player’s behalf. If equipment is requested Onward! A Legacy Foundation will contact you to coordinate.
Eligibility is determined based on the following gross income guidelines:

Valid forms of proof of income include

Copies of 2 recent pay stubs
Copy of notice stating amount: SSI, Social Security, VA, Disability (SSDI) Pension, Unemployment benefits, Worker’s Compensation Proof of Child Support Payment, Welfare benefits AFDC or any other income
SSI, Social Security, VA, Disability (SSDI)
Pension, Unemployment benefits, Worker’s Compensation
Proof of Child Support Payment, Welfare benefits
AFDC or any other income
Copy of a bank statement showing direct deposit of such payments
Copy of court order or other proof of child support amount
